Session 2: MaHIMA Webinar - Telehealth Documentation Best Practice

Session 2:  Wednesday – December 1,  2021

12:00pm - 1:00pm EST

Summary: Telehealth provider documentation best practice.   How can a provider best engage a patient in an audio/video or audio only Telehealth visit?

Objectives:  The objective is to update all attendees on the importance of accurate documentation to support medical necessity during a Telehealth visit. How can providers help engage patients during a Telehealth visit to capture the reason for the visit and complexity involved in the Medical Decision Making (MDM) that ultimately supports E/M leveling. Documentation is just as important in a Telehealth visit as in face-to-face visits.  Reporting whether the visit was by video and audio or just audio is important.  The reason for the visit, discussion of the visit as well as assessment of the evaluation and plan should be provided in the note.  It should be clear who provided the service and with patient consent. We will discuss the requirements necessary to meet during a Telehealth visit and how as in face-to-face visits medical necessity is supported by documentation. Establishing a chief complaint warranting medical evaluation, description of discussion and any medications and/or orders. We will review the importance of Chronic Condition capture and documentation best practice.
