This session will propose a clinical documentation improvement educational framework that clearly differentiates and defines appropriate documentation of encephalopathy versus drug-induced delirium, psychosis, coma, and intoxication as well as sundowning, withdrawal, and ICU delirium.
This presentation will begin with a physician’s perspective on why clinicians value the term, “altered mental status” (AMS) and why they resist CDI queries for “encephalopathy.” The session will then explore the overlapping AMS definitions, especially for delirium vs encephalopathy. Finally, the session will propose a CDI educational framework that audience members can take back to their facilities, as a long overdue guide for documenting all the major terms under “altered mental status.”
